------------------------------------------------------------------------------------------------------------------- Subject in Focus Managing Incident Reporting System
INCIDENT- An incident is an unfavourable event that affects patient or staff safety. Example- Physical injuries, Medical errors, Equipment failure, Administration, Patient care, etc REPORTING SYSTEM - Process of collecting incident data and presenting it properly. • Problem highlighted in a non-blaming way.
• Designated staff with authority or staff who has witnessed an incident first
• Usually, nurses or other hospital staff file the report
• 24 to 48 hours after the incident occurred: outcomes improve as the memories of the event are still fresh.
Patient Incidence Type:
Purpose of Incident Reporting
• To communicate information to Administrators.
• Information sheds light on measures that need to be taken to provide effective patient care as well as keep the facility running smoothly.
Reports helps Administrator with:
• Risk management - Knowing that an incident has occurred leads administrators to correct actions - This reduces the risk of similar incidents in the future.
• Quality control - Medical facilities want to provide the best care and customer service possible. Reviewing incident reports reveals areas that could be improved.
• Training - Using resolved patient incident reports to train new staff helps prepare them for real situations and current staff to learn from their own or others’ mistakes preventing incidents from reoccurring.
• Legal evidence - Should a patient take legal action following their incident, a thorough incident report is the most important part of any defence. Thus, all reports should be timely, complete and accurate.
Incident Reporting System
Incident Reporting Checklist
Drawbacks •Time, skill, and energy required is massive.
• Reporting everything: overburdens the staff.
Solutions
• Better to generate fewer reports, and effectively act upon them.
• Every medication error or every fall need not be reported.
•Sampling error categories may be introduced; Jan is falls month, Feb is pressure bed sores month etc.

Workforce Issues in a Healthcare Set-up
The most important resource of the any organisation is its human resource.
The Health Workforce refers to all of the people who deliver or assist in the delivery of health services or help operate health care facilities i.e. clinical staff, such as physicians, nurses, pharmacists and dentists as well as management and support staff.

This contributes to nurses being stretched thin and overworked. - Non-nursing Role: In almost all healthcare settings, nurses undertake roles which are not of their forte, for e.g., billing, record keeping, inventory, laundry, diet, physiotherapy, absconding of patient, etc., thereby diminishing time for patient care.
