---------------------------------------------------------------------- Triage • It is a process of prioritising patients based on the severity of their condition so as to treat as many as possible when resources are insufficient for all to be treated immediately. • In case of any disaster where a number of people are affected in the neighbourhood, hospital takes the moral responsibility to accept the patients and provide first aid or the basic treatment. The maximum number of patients can be treated is limited to a particular number of which the number of unstable patients requiring life support is limited to a particular number. • Other elective admission for patients is stalled until the situation comes to normal. • Triage shall be initiated whenever faced with a situation of mass casualties. • If faced with a large number, casualties need to managed priority wise • The aim is to ‘do the best for the most’ ---------------------------------------------------------------------- Triaging:- Triaging of the patients is done at the main lobby by any of the doctors who are on duty. During external disasters, patients shall be triaged as Red, Yellow, Green and Black according to the following criteria. Red First Priority, Most Urgent, Life-Threatening shock or hypoxia is present or imminent, but patient can be stabilized and if given immediate care, shall probably survive. Yellow Second Priority, Urgent, Injuries have systemic implications or effects, but patient is not yet in life-threatening shock or hypoxia, although systemic decline shall ensure, given appropriate care, patient seems able to withstand a 45 to 60-minute wait without risk Green Their Priority, Non-urgent, Injuries are localized and without immediate systemic implications, with a minimum of care, patient generally does not deteriorate for up to several hours. Black Dead, no distinction can be made between clinical and biologic death in a mass casualty incident, and any unresponsive patient who has no spontaneous ventilation or circulation is classified as dead. • These colored ribbon bands stickers are made available at the E.R room and Front Desk and the ICU. • The doctors and consultants at all levels and specialities shall act as treating doctors during this situation. • For detailed procedure on disaster management refer Safety Manual ----------------------------------------------------------------------
The quote highlights the crucial role of empathy and compassionate care in medicine. Patients may forget their doctor's name, but they will always remember how the doctor made them feel. This underscores the importance of emotional connection and patient-centered care in medical practice.
📚 Book I'm Reading This Week
Brief:- "The Checklist Manifesto" by Dr. Atul Gawande explores the profound impact of using checklists in various fields to improve efficiency, reduce errors, and enhance performance. Drawing on his experiences as a surgeon, Gawande illustrates how this simple tool can lead to significant advancements in complex environments such as healthcare, aviation, and construction. The book delves into the psychology behind why checklists work and offers practical advice on creating effective ones. Gawande's compelling narrative demonstrates that, despite our increasing reliance on technology, the humble checklist remains an essential instrument for achieving consistent and reliable results.
In 1906, William Osler emphasized the necessity of humility to medical students, a virtue that remains crucial today. Despite the pressures of modern medicine—global budgets, evidence-based approaches, and cost-containment—humility often goes undervalued. Bright, talented medical students can sometimes lack this essential trait. As they strive to become competent physicians, they must also learn to develop patient-physician relationships marked by humility.
Balancing technical skills with humanistic care is challenging but vital. Teaching humility, or "confident humility," is difficult but may be most effective at the bedside. Jack Coulehan, MD, MPH, identifies four attributes for 21st-century physicians: unpretentious openness, avoidance of arrogance, honest self-disclosure, and modulation of self-interest.
Methods for teaching humility vary. One approach involves panel discussions, home visits, and simulations. Another, more controversial, method focuses on acculturation, selecting medical students and professors who already display humility. Additionally, students are encouraged to self-reflect, recognizing their limitations and continuously learning.
The Stanford approach incorporates these ideas, fostering humility through practice and community. Clinical professors can support this by building confidence, asking thoughtful questions, pushing students out of their comfort zones, helping them improve, and modeling humility.
Ultimately, as Osler suggested, humility is a precious gift that enhances patient care and reduces medical errors.
As companies mature in this model, they can introduce new lines of business at incremental costs, which have the potential to generate substantial profits. These new ventures are supported by the foundation of previous low-margin businesses that have already captured customer loyalty and scale. A prime example is Amazon, which operates at slim to negative margins on products like Kindle devices and offers free shipping to Prime members but generates significant profits from Amazon Web Services (AWS), third-party merchandise, and advertising.
This model suggests that rather than focusing solely on traditional metrics like annual revenue or profit growth, one should also consider the strategic market placement, such as shelf space in the case of Colgate Palmolive. The company's established presence in retail outlets means that new products can yield high returns on capital due to existing brand recognition and network effects.
