ICU Care: Prehabilitation vs. Rehabilitation – Which Should Be the Norm?

What Is Rehabilitation?

Rehabilitation refers to helping patients recover their physical and mental abilities after illness or injury. In the ICU, rehabilitation typically starts after a patient has stabilized.

For example:

  • Patients are often mobilized (such as sitting up or walking) days after intubation or sedation.
  • Most ICU protocols wait for certain conditions, like ventilator weaning, before initiating movement.

However, delaying mobility can cause significant harm. Studies show that critically ill patients lose about 2% of their muscle mass per day during the first week of ICU stay. This can lead to long-term physical and cognitive impairments, making rehabilitation more challenging and increasing hospital stays.

What Is Prehabilitation?

Prehabilitation focuses on preventing physical and mental decline during critical illness by starting early interventions. Even though most ICU patients cannot predict their illness, prehabilitation involves proactive care as soon as they are admitted to the ICU.

This includes:

  • Early mobility: Moving patients within hours of intubation, unless contraindicated.
  • Sleep hygiene: Promoting restful sleep to reduce delirium.
  • Proper nutrition: Supporting the body to maintain strength.
  • Family involvement: Encouraging loved ones to support the patient’s mental health.

Studies show that mobilizing patients within 72 hours of ICU admission leads to better outcomes, such as:

  • Shorter hospital stays.
  • Reduced delirium rates.
  • Faster recovery to pre-ICU function.

Most ICUs follow a rehabilitation model, focusing on recovery after complications like ICU-acquired weakness or delirium. However, prehabilitation shifts the focus to prevention, avoiding complications before they develop.

In some ICUs that prioritize prehabilitation, patients are mobilized within 12 hours of intubation, drastically reducing delirium and improving outcomes. Nurses and clinicians report greater satisfaction seeing patients walk out of the ICU instead of being transferred to long-term care facilities.

How Prehabilitation Changes ICU Outcomes

  1. Delirium Rates Drop: Delirium, which affects up to 80% of ICU patients, becomes rare with early mobility and better sedation practices.
  2. Shorter Hospital Stays: Prehabilitated patients spend fewer days on ventilators and in the ICU.
  3. Improved Recovery: Patients regain physical and cognitive function faster, often avoiding long-term care facilities.
  4. Higher Staff Morale: Nurses and doctors feel fulfilled seeing their patients thrive and recover quickly.
